To make your visit as smooth and as pleasant as possible you can download / print the patient registration forms HERE and bring them to your appointment. Please arrive 15 minutes prior to your appointment time as this will give us sufficient time to make sure your records are complete with the information we need. Also, please bring:
  • Referral/pertinent medical records from your referring doctor
  • Identification and insurance cards
  • Prior authorization numbers - if required by your insurance
  • Co-payment
  • List of current medications and the dosages
  • Any laboratory tests, reports, dictations or handwritten summaries your doctor may have given you
Please allow three hours for your initial appointment as we can conduct many tests onsite. This will help us determine the health and medical needs of your eyes more quickly. Follow-up visits and postoperative visits usually last between one to one-and-a-half hours.

What to expect on your first visit

When you arrive please bring your previously completed registration forms if you accessed them through this site or if they were mailed to you. Also bring your insurance card and drivers license. If you did not previously complete the registration forms, plan on doing so at your visit by arriving 15 minutes early.
Your medical evaluation will begin with an ophthalmic technician talking with you to secure a more detailed medical and ocular history. Drops will be placed in your eyes to dilate your pupils. These eye drops can blur your vision, especially your reading vision, for several hours. Due to the dilating drops and subsequent bright lights shining in the eyes for evaluation, your vision may be blurry when you are done the examination. We strongly recommend that patients come with someone to drive them home. After your pupils are dilated, one of our doctors will meet with you to examine your eyes. Depending on your medical condition, other special tests may be performed. Because of the extended medical evaluation and possibility of additional testing, please allow 2 to 3 hours for your initial visit.
